ComForCare Home Health Care is seeking a Registered Nurse (RN) to support our home health patients and clinical team in the San Diego area. This role includes skilled nursing visits, start-of-care assessments, care coordination, clinical documentation, and support for LVN training and compliance activities.
The ideal candidate is organized, dependable, confident in the field, and committed to providing safe, high-quality care in the home setting.
Responsibilities

  • Complete skilled nursing visits, including start-of-care, resumption-of-care, recertification, discharge, and other assigned visits.
  • Perform patient assessments and help develop, update, and follow individualized plans of care.
  • Provide skilled nursing interventions, medication management, disease process teaching, patient education, and safety guidance.
  • Document clinical care accurately and on time, including OASIS, visit notes, care plan updates, and required records.
  • Communicate patient changes and care needs to physicians, families, and appropriate team members.
  • Support LVN orientation, field training, skills review, competency check-offs, and clinical coaching.
  • Review assigned documentation and field performance to support quality, compliance, and continuity of care.
  • Assist with chart audits, Medicare home health requirements, ACHC survey preparation, staff education, and corrective action follow-up.
  • Participate in case conferences, clinical meetings, QAPI activities, and performance improvement efforts.
  • Maintain patient confidentiality and follow agency policies, safety standards, and infection control procedures.
Qualifications

  • Current Registered Nurse license in California.
  • Graduate of an accredited school of professional nursing; BSN preferred.
  • Minimum of 1 year of nursing experience required.
  • Home health experience strongly preferred.
  • Experience with OASIS, start-of-care assessments, Medicare home health, ACHC standards, or survey readiness preferred.
  • Current CPR certification.
  • Valid drivers license, reliable transportation, and current automobile insurance.
  • Strong clinical judgment, communication, documentation, and time-management skills.
  • Ability to work independently in patient homes and travel within the service area.
  • Ability to train, support, and evaluate LVNs within applicable scope-of-practice standards.
Work Environment
This is a field-based home health position with visits to patient homes and agency locations. The RN may work in a variety of home environments and must follow agency safety, documentation, and infection control standards. Hours may vary based on patient care and agency needs.
