Duties:
- Performs Patient assessments: Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals. Develops the plan of care for the patient and family
- Ensures appropriate utilization of services: Maintains visit productivity standards.
- Demonstrates knowledge of insurance reimbursement Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours.
- Completes re-visit documentation and transfers Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools used
- Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
- Revises care plan as indicated.Prioritizes care needs.Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
- Shows evidence of interdisciplinary communication. Performs HHA supervisions per standards.
- Ensures appropriate utilization of services. Follows scheduling process. Communicates progress toward goals with IDT. Attends and participates in IDT meetings every 2 weeks.
- Evaluates the plan of care and revises as needed Maintains safe environment for patients and co-workers. Follows infection control measures and reports infections. Demonstrates knowledge of pharmacology and safe medication management.
- Routinely reconciles medications. Maintains current medication orders Attends monthly staff meeting:Participates in PI meetings and activities/committees as requested. Accepts other assignments as requested.
- Assumes responsibility for case management: Prioritizes schedule to accommodate patient's needs. Documents case management activities. Demonstrates knowledge of pharmacology and safe medication management.
- Routinely reconciles medications. Maintain current medication orders. Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities.
- Attends UR meetings as requested. Provides orientation/ mentorship to new employees. Cross trains to learn new skills and demonstrates a willingness to teach other
- Assists with growing agency business: Provides orientation to new employees. Accepts other assignments as requested. Cross trains to learn new skills.
Skills:
- Must have 2-3 years as RN in home hospice environment
- Must have valid New Jersey driver’s license
- BLS - required
- Experience using an EMR,
- excellent critical thinking and communication skills
