Job description

Duties:
  • Performs Patient assessments: Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals.
  • Develops the plan of care for the patient and family
  • Ensures appropriate utilization of services: Maintains visit productivity standards.
  • Demonstrates knowledge of insurance reimbursement Completes discharge assessments: Performs laptop transfer of data twice daily.
  • Completes admission assessment and transfers within 24 hours.
  • Completes re-visit documentation and transfers
  • Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary.
  • Documents patient education tools used Assumes responsibility for Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
  • Revises care plan as indicated.
  • Prioritizes care needs.
  • Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
  • Shows evidence of interdisciplinary communication.
  • Performs HHA supervisions per standards.
  • Ensures appropriate utilization of services.
  • Follows scheduling process.
  • Communicates progress toward goals with IDT.
  • Attends and participates in IDT meetings every 2 weeks.
  • Evaluates the plan of care and revises as needed
  • Maintains safe environment for patients and co-workers.
  • Follows infection control measures and reports infections.
  • Demonstrates knowledge of pharmacology and safe medication management.
  • Routinely reconciles medications.
  • Maintains current medication orders Attends monthly staff meeting: Participates in PI meetings and activities/committees as requested.
  • Accepts other assignments as requested.
  • Assumes responsibility for case management: Prioritizes schedule to accommodate patient's needs.
  • Documents case management activities.
  • Demonstrates knowledge of pharmacology and safe medication management.
  • Routinely reconciles medications.
  • Maintain current medication orders.
  • Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities.
  • Attends UR meetings as requested.
  • Provides orientation/ mentorship to new employees.
  • Cross trains to learn new skills and demonstrates a willingness to teach other Assists with growing agency business: Provides orientation to new employees.
  • Accepts other assignments as requested.
  • Cross trains to learn new skills.
Skills:
  • Must have 2-3 years as RN in home hospice environment
  • Must have valid New Jersey driver’s license
  • BLS - required
Education: 
  • Current New Jersey RN License Experience using an EMR
  • Excellent critical thinking and communication skills
Schedule Notes:
  • This role would be for the evening hours 3p -11p and would require every other weekend commitment.
  • Comp days would be provided during the week when working the weekend. T
  • raining for the position would require the candidate work on the day shift until cleared to work independently.
  • A hospice experienced nurse to work across our 3 hospitals who would be able to admit patients to GIP (general inpatient) level of care and provide medication recommendations to our hospital clinicians to manage end of life symptoms
