Job Purpose:
Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN will report directly to the Director of Nursing.
Job Responsibilities:
  • Explaining services by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, community stakeholders on Home Care Services and the admission process.
  • Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.
  • Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.
  • Provide nursing assessments and patient education.
  • Treat wounds, change wound dressings, giving injections, administer enema , and diabetic foot checks
  • Collect specimens from patients
  • Administers medications and set up medication dispensers.
  • Measures and records vital signs
