Job description

Liquid Personnel is recruiting an Occupational Therapist - Reablement at Home to join its client's Reablement at Home team, based in Hackney.

This role sits within a therapyled reablement pilot service, designed to inform future recommissioning of wider homecare services. The focus is on supporting adults to regain independence at home following hospital discharge.

What will your responsibilities be? 

  • Deliver shortterm reablement interventions (up to 6 weeks)
  • Support service users to regain or retain daily living skills
  • Work from Reablement Therapy Plans provided at referral
  • Promote strengthsbased, outcomefocused practice
  • Use community resources beyond personal care
  • Gradually reduce support as goals are achieved

  • Enable independence and autonomy
  • Reduce or avoid dependency on longterm services
  • Deliver care closer to home and at the right time 

Qualifications and Experience: 

To be successful in this role you must have;  

  • HCPCregistered Occupational Therapist
  • Experience in reablement, discharge or community OT
  • Strong personcentred assessment skills
