Job Summary and Responsibilities

We are seeking a skilled and compassionate Physical Therapist to join the VNA Health at Homeย team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.ย 

The focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most โ€œfunctionalโ€ place to make an impact on this!

Responsibilities:ย 

  • Conduct evaluations of patientsโ€™ physical abilities, mobility, strength, and functional limitations.ย 
  • Implement therapeutic exercises, manual therapy techniques, and modalities to improve patientsโ€™ physical function and mobility.ย 
  • Educate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
  • Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.ย 
  • Maintain accurate and up-to-date patient records in compliance with state regulations and agency policies.ย 
Job Requirements
  • Bachelorโ€™s degree from an accredited school approved by the American Physical Therapy Association
  • Active registered PT license in the state which you practice
  • Must obtain and maintain current CPR/BLS Certificationย 
  • One-year physical therapy experience in teaching or clinical setting is preferred, but not required.ย 
  • Current Drivers License
  • Comply with federal, state and local laws and regulations
