Home Health REGISTERED NURSE (RN) CASE MANAGER

Full Time

Job Description:

  • Responsible for care and documentation of care which meets reimbursement guidelines.
  • Coordinate physical care of the patient with family, patient, physician and Hospice team.
  • Communicate with patients/families at all levels.
  • Communicate with all agency staff.
  • Willingness to travel within the southside to see/meet with patients and their family.
  • Minimal call volume.

Job duties:

· Follows standard Nursing practices and implements agency policy and procedures.

· Assesses patient’s physical and emotional status, safety at admission and on an ongoing basis.

· Develops comprehensive care plan based on each patient’s assessment.

· Provides direct care and gives instruction to nursing assistants.

· Records and reports changes in condition to the physician.

· Participates in IDT.

· Documents orders in accordance with regulatory and agency requirements.

Job Requirements:

  • Current RN licensure in state of practice, successful completion of a nursing education from an approved school of nursing with one year of clinical nursing experience and at least one year of case management experience. Hospice nursing experience is ideal.
  • Knowledge of the framework, function, and organization of a home health and hospice care program and the nurse’s role as a member of the patient care team.
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
