Home Health PRN Registered Nurse (RN)

Are you an experienced Home Health RN looking for flexibility, autonomy, and the opportunity to make a meaningful impact in your community?

Elara Caring is seeking compassionate and skilled Registered Nurses to join our growing Home Health team on a PRN basis. In this role, you'll enjoy the independence of managing your own schedule while providing one-on-one care to patients in the comfort of their homes.

As a Home Health Registered Nurse, you will manage a caseload of patients and provide comprehensive nursing care throughout their healthcare journey.

Responsibilities include:

  • Completing skilled nursing assessments and developing individualized plans of care
  • Performing admissions, routine visits, recertifications, and discharge visits
  • Evaluating patients' physical, psychosocial, environmental, and educational needs

* Current, unrestricted Massachusetts Registered Nurse (RN) license* Minimum of one (1) year of nursing experience in a clinical care setting* Ability to commit to at least one full day (8 hours) per week* Valid driver's license, reliable transportation, and current auto insurance* Willingness to travel throughout the assigned territory

* Previous Home Health experience strongly preferred* Hospice experience preferred* Experience with HomeCare HomeBase (HCHB) preferred* Associate or Bachelor's Degree in Nursing

* Physical Requirements -Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
