Job Overview

Visiting Nurse Health System is hiring a Physical Therapist who brings not just clinical skill, but warmth, empathy, and a calming presence to every patient interaction. This role is more than just a job, it’s a chance to walk alongside our patients during some of life’s most meaningful moments. We're seeking a Physical Therapist who believes physical therapy is their calling in life and understands the importance of patient care. At Visiting Nurse, we strive to ensure every patient is treated with dignity, comfort, and care.

What You’ll Do:

  • Perform initial physical therapy assessments for home health patients
  • Develop and revise individualized treatment plans and discharge plans
  • Coordinate with interdisciplinary team members to create individualized care plans
  • Deliver compassionate, skilled therapy, including preventative and rehabilitative techniques in the home setting
  • Educate patients, families, and caregivers on care techniques and recovery to ensure comfort and understanding
  • Document patient progress, physician orders, therapy outcomes, and maintain accurate records in compliance with regulations
  • Supervise PTAs and HHAs, including conducting supervisory visits
  • Comply with Medicare, Medicaid, and insurance regulations
  • Drive to patient homes and work independently in the field
  • Provide emotional support to patients and loved ones during challenging times

Schedule:

  • Monday to Friday
  • 8am – 5pm
  • Scheduled Visits

Location:

  • Stone Mountain, GA (based out of our Dunwoody office)

You’re perfect for the position if you have:

  • Doctorate from an accredited Physical Therapy program
  • Current GA Physical Therapy license
  • CPR certification and valid driver's license required
  • Reliable transportation and automobile insurance
  • Experience with Medicare/Medicaid regulations and compliance in home health
  • Strong documentation and assessment skills (e.g., OASIS, care plans, discharge planning)
  • Ability to educate patients, families, and interdisciplinary team members
  • Competency in managing rehabilitative therapy services and patient care protocols
