Home Health Caregiver

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on -- providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Licensed HHAs/CNAs are encouraged to apply -- and we also welcome motivated candidates willing to become certified.

Key responsibilities

  • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
  • Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
  • Prepare meals and assist with feeding as needed
  • Support medication self-administration through reminders (no independent med administration)
  • Monitor and report patient condition and changes to the supervising nurse
  • Take and record basic vital signs when trained and assigned
  • Maintain a clean, safe, and orderly client environment
  • Assist with linens and light client laundry as appropriate
  • Use safety equipment properly (rails, supports, stools, etc.)
  • Document care provided according to company and regulatory standards
  • Travel reliably to patient homes and assigned visits across your service area
  • Complete required in-service training, including annual dementia education

Must-have requirements

  • Age 18 or older
  • Active HHA or CNA certification or willingness to obtain certification
  • Willingness to complete competency and skills assessments
  • Ability to follow written care instructions and document services
  • Compassion, patience, and professionalism with vulnerable populations
  • Ability to work independently in home settings
  • Reliable transportation and ability to travel to client homes
  • Ability to lift up to 25 pounds
  • Willingness to complete TB testing, background screening, and onboarding requirements

Nice-to-have

  • 6+ months of caregiving or direct patient care experience
  • Home health or home care experience
  • CPR and Adult First Aid certification
  • Experience supporting seniors or medically complex clients
  • Experience working with dementia or memory care patients

Work environment & schedule

  • One-on-one care in patient homes and occasional facility settings
  • Flexible scheduling options may be available (full-time, part-time, PRN depending on market)
  • Assignments may include homes with pets or smokers
  • Travel within your preferred coverage area
