IMPORTANT TO NOTE: THIS IS A DAY SHIFT POSITION, HOURS WILL BE 8AM-5PM

Our Home Health Aides have been called to care when they’re needed most. As a member of our home health and hospice teams, you’ll support a full range of patient services to bring comfort and dignity to our clients.

The Home Health Aide:

  • Assist patients with ambulation and transfer using proper body mechanics.
  • Communicate any change in condition or lack of patient progress.
  • Complete necessary documentation of care provided.
  • Record intake, output, and vital signs as directed and communicate care as appropriate.
  • Provide assistance with personal hygiene and elimination needs according to the established plan of care.
  • Schedule visits with the patient or caregiver.
  • Assist patients with care and activities of daily living as directed in the established Plan of Care.
  • Current HHA or CNA licensure.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Ability to manage your schedule effectively and efficiently.
  • Ability to work in a field setting and to make sound nursing judgements.
  • Ability to assess patient needs and formulate individualized patient care plans to meet those needs.
