Job description

The Clinical Director / Director of Patient Care Services is responsible for the overall leadership and direction of home health clinical services. This role establishes, implements, and evaluates clinical goals and objectives that support quality patient care, regulatory compliance, and the overall mission and philosophy of the organization.

The Director of Patient Care Services also serves as the Clinical Manager for the purpose of meeting Medicare Conditions of Participation. This position provides clinical oversight, operational guidance, and leadership to ensure home health services are delivered safely, effectively, and in compliance with all applicable local, state, federal, and accreditation standards.

Required Education and Experience

  • Degree in Medicine or Bachelor's degree in Nursing from an accredited program by the National League for Nursing
  • Additional years of relevant experience may be considered in lieu of education
  • Three years of management experience within the last five years in a home health or related healthcare organization, with at least one year in a supervisory or administrative capacity

Alternative Qualification Pathway

The Clinical Director / Director of Patient Care Services may also qualify as a registered nurse with four years of experience within the last five years in a home health agency, primary care clinic, or health facility, with at least one year in a supervisory or administrative capacity.

Preferred Education

  • Master's degree in healthcare or related field preferred

Required License

  • Current Registered Nurse license to practice professional nursing in the state of California, or current physician license in the state of California
  • Provide overall direction and oversight of home health clinical services
  • Establish, implement, and evaluate goals and objectives for home health services
  • Coordinate and oversee all direct and indirect patient care services provided by clinical personnel
  • Serve as the Clinical Manager for purposes of Medicare Conditions of Participation
  • Provide guidance, coaching, and support to Clinical Supervisors, Nursing Supervisors, and coordinators
  • Support clinical staff in assessment, planning, implementation, and evaluation of patient and family/caregiver care
  • Monitor operational indicators, including census changes, volume trends, staffing needs, revenue impact, and expense considerations
  • Identify changes in patient volume that may impact staffing levels or operational performance
  • Ensure compliance with all local, state, and federal laws related to licensure and certification of organization personnel
  • Maintain compliance with CHAP Home Care standards
  • Promote continuous improvement in all aspects of home health clinical services
  • Perform other duties as assigned
