Essential Duties and Responsibilities:
- Supports adherence to HSL policies and procedures and provides guidance to communities on policy implementation. Assists with policy and procedure development and modifications as needed
- Educates, staff, managers and leaders of assigned communities on quality improvement processes
- Works directly with assigned community leadership teams to monitor indicators, dashboards and performance improvement projects. Tracks, analyzes and reports on all PIPs to Quality team
- Assists assigned communities with implementing, monitoring and tracking of resident, family and employee satisfaction surveys
- Implements and monitors community action plans
- Facilitates performance initiatives at assigned communities and assists with plans of correction as needed
- Complete quality indicator tracking and trending reports. Support community leaders on quality initiatives
- Monitor compliance in accordance to Heritage Senior Living policies, best practice protocols, and state and federal regulatory requirements
- Provide education to staff on electronic health record system to ensure compliance with documentation standard
- Participates in and monitors new company programs and pilots as necessary
- Act as interim DON depending on the needs of the organization
- Manage the training and the onboarding of DONs
- Assist with the Corporate Onboarding which includes, but is not limited to orientation & CBRF trainings
- Partners with the Vice President of Clinical & Quality Operations to develop, implement and evaluate annual infection prevention goals and action plan
- Partners with facility leaders, physicians, local, state, and national agencies on activities related to infection prevention
- Oversees the operations of the infection prevention, epidemiology, industrial hygiene, and relevant safety programs
- Accountable for surveillance of healthcare acquired and community acquired infections
- Ensures that education and counseling on infection prevention is available for staff, volunteers, medical staff, patients and parents.
Education/Qualifications
- Valid state of Wisconsin RN licensure
- Must be in good standing on the State Board of Regulation and Licensing
- Three years of nursing experience working with seniors; experience within an assisted living, memory care or other long-term care environment preferred
- Must be self-directed and able to function independently
- Managerial experience preferred
- Excellent interpersonal skills – ability to connect with residents, families, and associates
- Strong customer service orientation to older adults, exhibit a caring and compassionate attitude
- Ability to make responsible choices and decisions that are in the best interest of the residents and caregivers
- Ability to work collaboratively with all coworkers
- Excellent written and verbal communication skills; must be proficient in Microsoft; PointClickCare software experience a plus
- Ability to problem solve and communicate complex issues
- Must submit to all applicable pre-employment and annual tests per state and federal regulations
- Must exhibit a caring and compassionate attitude while articulating true concern for people
