Job description

Eliot is seeking a strong, experienced,  licensed RN, Nurse Case Manager, to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.

The ideal candidate would  possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.  

Responsibilities:

  • Act as a primary case manager to 3-6 consumers
  • Provide nursing interventions to individuals
  • Facilitate and coordinate medical care to include communication with health care providers
  • Provide health education to individuals 
  • Assist in the management of medications and the overall functioning of the nursing department
  • Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
  • Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
  • Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients

Qualifications:

  • Requires an associates degree in nursing
  • LPN or RN nursing license
  • Experience working with adults with mental illness and knowledge of psychiatric and medical medications
  • Valid Massachusetts drivers license 

Schedule: Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall schedule 

Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase
