Role Overview

We're seeking experienced Primary Care Nurse Practitioners to provide on-site care to residents in Assisted Living and RCFE communities across the Inland Empire. If you're passionate about geriatric care and want to make a meaningful difference in the lives of seniors - without the walls of a clinic - this role is for you.

What You'll Do
  • Conduct routine and urgent assessments; manage chronic disease across a dedicated patient panel
  • Perform post-discharge follow-ups, medication reconciliation, and coordinate labs and diagnostics
  • Collaborate with facility staff, specialists, families, and POAs to ensure seamless continuity of care
  • Complete same-day documentation in compliance with CMS and regulatory standards
  • Serve as the primary on-site clinical provider, building lasting relationships with residents and care teams
Schedule & Setting

Part-time position with a flexible weekday schedule. Providers deliver on-site care across the Inland Empire with strategically clustered routes - so you spend more time with patients and less time on the road.

Requirements

  • Active California NP and RN licenses; active furnishing license
  • Valid NPI, DEA, and BLS certification
  • Bilingual fluency in Spanish and English required
  • Experience in geriatrics, assisted living, RCFE, or facility-based primary care preferred (strongly preferred)
  • EMR proficiency and commitment to same-day documentation
  • Comfortable working independently in a mobile, field-based environment
