Job Summary:

Position is primarily responsible for providing clinical social work to ensure a full range of services to promote general social and emotional well-being to the residents and their families throughout the continuum.

The position is Monday-Friday 8:30am-5:00pm

The pay range for this role is $80k to $90k

Job Duties:

  • Interviews potential Assisted Living residents to compile a social history and determine level of care.
  • Offers direct social work interventions to residents and their families and provides appropriate measures for helping residents in their personal adjustment, providing emotional and social support and accessing the appropriate resources.
  • Counsels with residents to enable them to discuss and understand their feelings, attitudes, fears and anxieties related to problem solving and to explore alternative solutions.
  • Follows NASW Code of Ethics in daily work.
  • When needed, in absence of HC Social Worker, coordinates services to HCC residents with medical and nursing staff to include care plans, MDS, documentation as required by the Department of Health and DSS. Collaborates with staff to complete the Minimum Data Set (MDS) within 14 days of resident admissions, quarterly, annually, and as required by change in resident status, and updating information upon any change in resident status.
  • Plans and coordinates Individualized Service Plans for residents upon admission, at least on an annual basis and when there is a significant change, with the Interdisciplinary Team, resident and family members.
  • Facilitates the weekly Inter-Disciplinary Team meeting to determine the appropriate plan of care for AL. Provides quality resident care by coordinating and facilitating conferences with families/POA and other interested parties to address problems of adjustments, treatment and care-giving issues.
  • Plans and provides clinical supervision when needed for student interns assigned to the department.
  • Provides follow-up Social Work visits to newly admitted AL residents and documents such visits within ninety (90) days after arrival in resident’s administrative file.

Job Qualifications:

  • Prefer a graduate degree in social work from an accredited university.
  • Prefer a Licensed Clinical Social Worker.
  • Must possess some computer skills.
  • Prefer to have at least three (3) years experience in social work with at least one (1) year in work with the geriatric population. Some supervisory experience preferred.
  • Must possess ability to deal tactfully and effectively with residents, staff, family members and the public.
  • Must meet regular and predictable attendance requirements.
