Nurse Practitioner/Physician Assistant - Adult Medicine

Virtus Healthcare Staffing is actively seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our partner's Adult Medicine team in Hartford, CT. This facility provides comprehensive medical, behavioral, dental, and pharmaceutical services under one roof striving to serve as a true medical home for the surrounding community. You will play a key role in providing individualized, culturally competent care and fostering lasting relationships with patients.
Role Summary
As a Nurse Practitioner or Physician Assistant in the Adult Medicine clinic, you will offer comprehensive health assessments, disease prevention, and chronic condition management. You will collaborate closely with physicians and interdisciplinary teams to ensure every patient receives timely, respectful, and high-quality care. Your services will contribute to a model that emphasizes prevention, early intervention, and continuity of care.
Key Responsibilities
  • Provide primary and urgent care services, including health screenings, exams, and diagnostic interpretation.
  • Maintain and document thorough patient histories, assessments, treatment plans, and follow-up care in the Electronic Health Record (EHR).
  • Collaborate with physicians, specialists, and ancillary staff to ensure coordinated, holistic care.
  • Make evidence-based decisions aligned with departmental protocols and organizational quality measures.
  • Refer patients for specialty services or diagnostics as appropriate.
  • Deliver culturally sensitive care and education to patients from diverse backgrounds.
  • Participate in performance improvement initiatives and team-based care meetings.
Required Qualifications
  • Graduate of an accredited NP or PA program.
  • Active State of Connecticut license as a Nurse Practitioner or Physician Assistant.
  • Eligible for DEA and State of Connecticut narcotic registration.
  • At least 1-2 years of clinical experience in an ambulatory or urgent care setting preferred.
  • Strong skills in patient communication, clinical assessment, and documentation.
  • Proficiency using EHR systems and digital documentation tools.
  • Ability to work independently while being a collaborative team player.
  • Must pass a pre-employment background check and drug screening.
