Now Hiring: Full-Time Social Worker Assistant

A compassionate and resident-focused Skilled Nursing and Long-Term Care Facility near Jenkintown, PA is seeking a Full-Time Social Worker Assistant to join our interdisciplinary team. This position is ideal for someone who is passionate about supporting residents and their families while helping ensure a positive experience throughout their stay.

The Social Worker Assistant will work closely with the Social Worker Director to provide emotional support, assist with discharge planning, coordinate community resources, and advocate for residents' psychosocial well-being.

Schedule
  • Full-Time

  • Monday–Friday

Social Worker Assistant Responsibilities
  • Assist the Social Worker Director with resident assessments, care planning, and discharge planning.

  • Provide emotional support and advocate for residents and their families.

  • Help coordinate admissions, transfers, discharges, and resident transitions.

  • Assist residents and families with accessing community resources and support services.

  • Participate in interdisciplinary care plan meetings.

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.

  • Address resident and family concerns while promoting resident rights and quality of life.

  • Collaborate with nursing, rehabilitation, admissions, and other departments to ensure continuity of care.

  • Perform other duties as assigned.

Social Worker Assistant Qualifications
  • Associate's or bachelor's degree in social work, Human Services, Psychology, or a related field preferred.

  • Previous experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, or healthcare setting preferred.

  • Knowledge of discharge planning and community resources is a plus.

  • Strong communication, organizational, and interpersonal skills.
