Registered Nurse (RN) โ€“ Hospice Home Health Case Manager

We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities.

The RN Case Manager will coordinate and manage care for patients with end-stage illnesses, ensuring comfort-focused care in various home-based settings. This role requires strong clinical judgment, excellent communication skills, and the ability to provide holistic care to patients and their families.

Patient Population:

  • End-stage hospice patients
  • Care provided in:
    • Patient homes
    • Assisted Living Facilities (ALF)
    • Skilled Nursing Facilities (SNF)
    • Group homes
    • Independent living communities
    • Two hospitals providing General Inpatient (GIP) hospice care

Unit Overview:

  • Average Daily Census: ~105 patients
  • RN Case Managers: 7 (each managing 14โ€“16 patients)
  • CNA Support: 6 CNAs

Key Responsibilities:

  • Manage a caseload of hospice patients in the community
  • Provide comprehensive nursing assessments and symptom management
  • Develop and implement individualized care plans
  • Coordinate care with physicians, hospice staff, and interdisciplinary teams
  • Educate patients and families regarding disease progression and end-of-life care
  • Document patient care accurately in the EMR system
  • Provide compassionate support to patients and families during end-of-life care

Requirements:

  • Minimum Experience: 2 years RN experience
  • Travelers: Experienced travelers preferred
  • Licensure: Active Registered Nurse (RN) license
  • Certifications:
    • BLS (Basic Life Support)
  • Other Requirements:
    • Valid Driver's License
    • Auto Insurance
    • Reliable transportation for community visits

Skills & Competencies:

  • Hospice or end-of-life care experience preferred
  • Strong patient assessment and care coordination skills

  • Shift: Monday โ€“ Friday, 8:00 AM โ€“ 4:30 PM

  • Floating: Community-based patient home visits
  • Orientation: 1 week
