Youth Case Manager
Location: Grayslake, IL (On-site / Community-based)

About the Role

Clarvida is seeking a Youth Case Manager to work with minors placed in care by DCFS due to abuse or neglect. In this role, you will partner with youth and families to develop individualized treatment plans with the ultimate goal of permanency. You will help identify service needs, coordinate wraparound community services, and assess progress toward stability and well-being.

This position includes regular contact with youth and families, observation of family visitation, and collaboration with legal teams, DCFS, and community providers. You will receive consistent, supportive supervision from a Child Welfare Supervisor to support your professional growth and development.

Responsibilities

● Meet with youth clients 1–3 times per month
● Meet with family members at least once per month
● Observe family visitation monthly
● Develop and implement individualized treatment plans focused on permanency
● Identify goals, strengths, and service needs for youth and families
● Coordinate wraparound services within the community
● Assess and document client and family progress
● Provide transportation for visits, medical appointments, and meetings as needed
● Collaborate with DCFS staff, legal teams, service providers, and families
● Maintain accurate, timely, and compliant case documentation

Requirements

● Bachelor’s Degree in a Human Services–related field
● CWEL license preferred
● Valid Driver’s License in good standing and active automobile insurance
● Ability to obtain an Illinois Driver’s License within 30 days of hire if relocating from out of state
