Job description

Job title: Case Manager: Internal Case Manager and VA Case Manager

Reports to: Director of Internal Case Management

FLSA Status: Full-time/Exempt

The Internal Case Manager and VA Case Manager is responsible for direct case management services of the VA Per Diem dorm and GATES dorm. These services include the coordination of community resources that support and promote long-term housing needs and should place and emphasis on the homeless community and Veteran community. These programs develop sustainable, quality driven programming which focuses on the individuals housing goals and allows the Internal Case Manager and VA case manager to provide a direct linkage to Veteran, mainstream, and community resources for participants. The desired candidate will have knowledge of the challenges faced by the Veteran community and recovery supportive services within the Milwaukee area. The Internal Case Manager and VA case manager will also provide collaboration with internal and external partners including Guest House Fund Development staff and VA Liaison. The Internal Case Manager and VA case manager will also participate in Comprehensive Service Team (CST) meetings, community networking and collaboration events, and represent the agency at Veteran Initiative meetings.

Essential Duties and Responsibilities

  • Manage a case load of up to 20 clients.
  • Provide full case management services for GATES Program and VA Per Diem program, including implementation of Individualized Service Plans (ISP), administrating assessments to determine client needs, and file maintenance.
  • Work directly with participants to progress towards income, health, and housing sustainability by directly assisting or providing referrals to community providers/services. Complete necessary follow-up or actions to assist clients in achieving their goals and meeting their needs.
  • Utilize community and veteran resources that provides services to Grant Per Diem clients such as job-readiness, literacy and GED/diploma programs, AODA and Mental Health treatment, counseling, benefits, drivers license recovery and housing.
  • Provide resources and referrals for a wide range of community-based programming and services to aid in the self-sufficiency, self-advocacy and overall improved quality of life.
  • Track participation, referrals, progress towards housing and other key elements within the program design.
  • Create and distribute workshop/activity schedules and ensure appropriate record keeping and reporting.
  • Accurately enter data in the agency database and Clarity in a timely manner.
  • Participate in the facilitation of VA/emergency shelter dorm meetings and weekly meetings with the VA liaison.
  • Participate in discharge planning and care coordination to assure that appropriate high-quality care is available to the resident upon exit from the program.
  • Some after-hour availability is required to respond to after-hour emergencies.
  • Other duties as assigned by the Director of Internal Case Management.


Qualifications/Training/Experience

  • Knowledge and experience utilizing community services, working with Veterans, housing resources and HUD funded housing.
  • Bachelor's degree in human services or related field plus two (2) years of case management experience working with diverse populations preferred.
    • In lieu of a bachelor's degree, must possess:
      • An associate degree in human services or related field; and
      • And a minimum of four (4) years of case management experience including experience in housing navigation or working with homeless populations.
  • Familiar with issues related to homelessness, mental illness, and substance abuse.
  • Understanding of motivational interviewing and stages of change principals preferred.
  • Maintain a valid Wisconsin driver's license, vehicle insurance, and reliable transportation in compliance with agency requirements.
  • Knowledge of federal poverty guidelines, HUD guidelines, and HUD rent calculations.
  • Knowledge of Clarity or other HMIS systems.
  • Crisis prevention and intervention training is provided but previous experience preferred.
