Job description

We are seeking a full time Program Coordinator for our location in Coon Rapids!
Program Coordinator responsibilities:
  • Assist the Program Manager in ensuring that the tasks on resident care plans are completed
  • Assist with schedule changes
  • Ensure completed documentation
  • Provide quality personal care services and health-related services

Program Coordinator Qualifications:
  • A two-year-degree in a social services area of study or an equivalent combination of experience and education
  • Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting
  • Ability to model excellent work behaviors and team-oriented problem solving
  • Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities
  • Ability to accompany and/or transport residents within the community
