Work Environment

This position requires an onsite presence, where you will directly engage with patients in their homes, fostering a friendly and supportive environment. The role allows you flexibility in managing your schedule Monday through Friday, as you develop meaningful relationships with your clients and their families. Key Responsibilities
  • Develop and implement individualized physical therapy treatment plans focused on preventing disabilities, relieving pain, restoring muscle function, and maximizing performance capabilities.
  • Supervise and guide Physical Therapy Assistants in following care plans, ensuring quality care delivery and adherence to therapeutic protocols.
  • Conduct thorough evaluations of patients' functional capabilities through various tests related to muscles, nerves, and joints, to inform treatment planning.
  • Assess the home environment to recommend modifications aimed at enhancing patient independence during their rehabilitation journey.
  • Administer therapeutic interventions, including manual therapy, exercise programs, and the application of physical agents such as traction and massage.
  • Evaluate and fit patients with prosthetic and orthotic devices, making necessary recommendations to their physicians.
  • Maintain accurate and up-to-date clinical records, including initial evaluations, care plans, progress notes, and discharge summaries.
  • Participate in interdisciplinary team meetings to discuss patient care and collaborate on treatment strategies.
  • Educate patients and their families on effective utilization of therapy devices and the overall physical therapy plan.
  • Complete documentation of clinical activities and process orders in compliance with company policies and maintain timely communication with team members.
  • Conduct on-site functional assessments and continuously review and revise care plans as necessary.
  • Be readily available to support Physical Therapy Assistants while they provide patient treatment.
  • Actively participate in organizational meetings and continued professional development opportunities.
Patient Population / Client Focus

Your primary focus will be on diverse patient populations receiving home care. This includes elderly clients recovering from surgeries, individuals with chronic pain or physical impairments, and patients needing rehabilitation after acute medical events. You will facilitate tailored therapy that addresses specific needs, promotes recovery, and fosters independence in each patient's living environment.

Qualifications
  • A Doctorate in Physical Therapy (DPT) is required.
  • Possession of a current and active Physical Therapist license in the state of New Jersey, with an impeccable record free from sanctions.
  • Demonstrated competency in manual therapy techniques and the ability to design successful exercise regimens.
  • Proficient use of electronic medical record (EMR) software, particularly Web PT, for accurate documentation of patient interactions and procedures.
  • Continuously updated knowledge of physical therapy practices backed by the latest research and empirical evidence.
Preferred Experience
  • Experience in a home care setting is highly valued.
  • Previous roles involving supervision or mentoring of Physical Therapy Assistants.

Compensation and Benefits
