Job description

) Coverage area: Ocean County - Toms River (08753), Brick, Jackson, Manchester and Whiting.
Provide high-touch palliative care for people facing serious illness across SNF, home, and community settings - with a structured ramp into palliative care, physician/APP backup, and a team that does not leave you to figure it out alone.
Base salary $130,000-$160,000/year plus annual performance bonus up to 12.5%. Primarily a Monday-Friday schedule, 28 paid time off days in year one, a $0/month employee-only medical option, malpractice coverage, license/DEA support, CME, and tuition support.
Setting • License: Outpatient - SNF & community, Ocean County NJ • New Jersey NP or PA license required
About us & the role:
NJ Palliative Care Solutions is on a mission to deliver palliative care without walls across New Jersey, partnering with skilled nursing facilities, home-based providers, and leading health systems. We are hiring a Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Ocean County for patients living with serious illness. Palliative care experience is helpful but not required - clinicians with strong communication and serious-illness judgment from oncology, critical care, hospital medicine, geriatrics, SNF/LTC, primary care, or home-based care should consider this role.
Why you'll want this role:
  • A structured ramp into palliative care: pair with experienced specialists and grow through mentorship, clinical backup, and data-guided coaching.
  • A real interdisciplinary team: physicians, APPs, social work, care coordination, and operational support.
  • Clear workflows and quality expectations - not a sink-or-swim field role.
  • Primarily a Monday-Friday schedule with flexibility to swap workdays.
  • 28 PTO days in year one and a $0/month employee-only medical option.
  • Academic pathways, CME up to $1,000/year, and tuition reimbursement up to $2,000.

What you'll do:
  • Perform at the top of your license, performing best practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine.
  • Provide palliative care consultations and follow-up visits across skilled nursing settings, including short-stay subacute/rehab patients with recent hospitalizations, high symptom burden, or complex goals-of-care needs, as well as ongoing management of long-term care residents.
  • Deliver palliative assessments and follow-up visits in SNFs and community settings.
  • Manage complex symptoms for patients with advanced illness and chronic disease.
  • Lead goals-of-care and advance care planning conversations with patients and families, coordinating with partner home health and/or hospice agencies if and when appropriate.
  • Partner with the interdisciplinary team and facility staff, primary care, and hospice.
  • Support transition planning across the SNF stay - from admission after hospitalization through discharge home, return to long-term care, rehospitalization decisions, hospice transition, or other next-site-of-care planning.
  • Complete timely documentation aligned with clinical, billing, and quality standards.
  • Travel locally within Ocean County, with mileage reimbursement and clinical/EMR support provided.

What you bring:
  • Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
  • DEA + CDS registration, or ability to obtain by start date.
  • 2+ years of clinical experience preferred; palliative care experience helpful, and high-potential APPs from relevant serious-illness settings considered.
  • Palliative care certification (e.g., ACHPN / CHPN) preferred.
  • Strong communication, clinical judgment, compassion, and comfort working on interdisciplinary teams.

Compensation & total rewards:
  • Competitive base salary: $130,000-$160,000/year.
  • Annual performance bonus up to 12.5% of base salary, rewarding both productivity (RVUs) and quality of care.
  • 28 days of paid time off your first year, growing to 33 days at 2 years, in one flexible bank.
  • Malpractice & professional liability ($1M / $3M), license & DEA renewals, up to $1,000/year CME, up to $2,000 tuition reimbursement.
  • Medical via Cigna PPO, including a $0/month employee-only option, plus dental, vision, and an HSA.
  • Short- and long-term disability, voluntary life & AD&D, and accident / critical-illness / hospital coverage.
  • 401(k) retirement plan, plus pre-tax FSA, dependent-care FSA, HSA, and commuter benefits.
  • 24/7 Employee Assistance Program, identity-theft protection, legal plan, pet benefits, and member discounts.

This position is full-time, and full-time is our strong preference. That said, we know exceptional clinicians sometimes need a different arrangement - for the right candidate, we're open to discussing part-time or per-diem schedules. If that's you, please apply and tell us what schedule you have in mind.
