The Assisted Living Supervisor is responsible for a supervisory role in the provision of assisted living services for residents of Deerwood Crossing Senior Residences, a licensed Residential Care Apartment Complex. Reporting to the Director of Assistant Living, responsibilities of the Assisted Living Supervisor include hiring, training, coaching, providing supervision and orienting Resident Assistant (RA) caregiver staff; scheduling and directing the daily duties and activities of RA staff; communicating with residents, family members, case managers, and others about resident care; scheduling medical appointments and managing the resident transportation calendar; administrative duties essential to RCAC operations; and other duties as assigned.
Responsibilities and Duties
We are seeking an organized, energetic and detail-oriented Assisted Living Supervisor to lead our assisted program. This key role combines clinical expertise with operational leadership to ensure exceptional resident care, compliance with regulations, and efficient facility management. Summary of position:
Under the guidance of the Director of Assisted Living, the Assisted Living Supervisor will coordinate and supervise day-to-day activities of the Lead and Resident Assistants, to ensure the delivery of quality, comprehensive services to Residents within the JFS Housing guidelines. They will work to protect the rights and safety of Residents/staff, observe, assess, and perform all duties as needed to provide care and services to Residents
Duties:
  • Oversee daily operations of Deerwood assisted living programs, ensuring high standards of resident care and safety.
  • Updates, implements, and monitors individualized care plans tailored to residents' needs, in conjunction with Director of Assisted Living and RN.
  • Supervises direct care staff, including recruiting, hiring, onboarding, orientation, and retention, fostering a collaborative team environment.
  • Ensures medication administration is conducted accurately and in compliance with all regulations.
  • Manages staffing schedules and fill in on floor as needed.
  • Manages clinical communication with outside entities, i.e. Pharmacies, doctors, nurses, care team, hospitals, rehabs, families, etc. and update Director and RN as needed.
  • Tracks and ensures proper documentation in medical record. (ECP)
  • Maintain compliance with state and federal regulations related to Deerwood RCAC license
  • Coordinates and assists with training of Resident Assistants in accordance with Deerwood Crossing policies and State regulations.
  • Conducts regular audits of task lists and medications administration records (MARs); to ensure that Resident Assistant tasks are completed properly and as scheduled;
  • Holds bi-weekly 1:1 meetings with all Resident Assistant staff and routes meeting notes for placement in employee file.
  • Serves on on-call rotation
  • Acts as a liaison between staff, residents, resident's families, physicians, Family Care team, and other healthcare providers;
  • Observes residents for emotional, behavioral, and physical changes in condition;
  • Reviews resident paperwork from medical appointments for changes in condition, med changes, and upcoming appointments;
  • Oversees resident transportation schedule and addresses gaps in transportation coverage as needed.
  • Provides excellent customer service to residents
  • Utilizes Nurse Call system and update system as needed;

Requirements
  • Licenced Practical Nursing (LPN) license with 2 years of LPN experience and 2 years managing a successful team is required;
  • 5-7 years of manager experience in lieu of licensure
  • Associates Degree required
  • Approachable, customer service orientation with sensitivity to a diverse resident population;
  • CBRF certified for medication administration, first aid/choking, fire safety, and standard precautions; current CBRF trainer or ability to be certified as a CBRF trainer a plus,
  • Excellent oral and written communication skills;
  • Knowledge of or ability to learn computer programs specific, including an Electronic Medication Administration Record system and a Nurse Call system;
  • Proficient in computer use and Microsoft Office applications;
  • Strong and demonstrated organizational skills;
  • Ability to serve on call for staff inquiries on a rotational basis;
  • Ability to occasionally work outside of regular work hours, as needed;
  • Ability to maintain resident confidentiality at all times.
  • Driving is a requirement for this role. Valid driver license and auto insurance required.
  • Knowledge, understanding, and/or willingness to learn Jewish culture and traditions and those of the Agency population as necessary;
  • Accept other duties and responsibilities as assigned by Director of Assisted Living or RN;
  • Candidates must successfully pass a criminal background check with the Wisconsin Department of Justice and a caregiver background check with the Wisconsin Department of Health Services, the Department of Children and Families and the Department Safety and Professional Services.
