Job description

Phoenix Service Corporation has a full-time Housing Service Assistant/Housing Case Manager position available. This position is 40 hours a week, Monday-Friday. No evenings, weekends or holidays!ย 

The Housing Service Assistant primary duties include:

  • assisting individuals with locating and gaining access to housing.
  • provides support and resources for medical, social, educational, vocational or other needed services regardless of funding source.
  • teaching independent living skills that maintain and enhance ability to live independently in the community.
  • requires frequent contact with individuals being supported and the ability to travel from location to location.
  • acts as a liaison between individuals and partnering agencies, including landlords.
  • participates as an interdisciplinary team member, functions as an advocate.
  • ideally has experience and knowledge navigating a variety of social service systems, including financial, housing, legal, county and health care services.
  • ideally has knowledge and experience working with a variety of disabilities and chronic illnesses, including but not limited to mental health diagnosis, traumatic and acquired brain injuries, intellectual disabilities, autism spectrum disorders and chemical addictions.
  • ideally has experience working with individuals who have or are experiencing homelessness.

The Housing Service Assistant must be able to communicate effectively with clients, families, outside agencies and coworkers. They must maintain and foster cordial relationships and optimal communication and displays positive, supportive and cooperative conduct. They must be able to communicate effectively both verbally as well as in writing; listening accurately and responsively. They must be able to work independently without direction or supervision immediately available. The Housing Service Assistant seeks/pursues services for non-English speaking clients as appropriate.

Requirements:

  • At least 1 years of experience with person with disabilities and/or in Health and Human Services.
  • Experience with person centered planning approaches.
  • Knowledge of disabilities/chronic illnesses.
  • Knowledge of independent living skills management.
  • Excellent communication skills, written and verbal.
  • Ability to coordinate activities and services.
  • Ability to read, write and speak the English language.
  • Ability to travel among agencies; holds a valid license and maintains a safe driving record.
  • Ability to work with diverse clients.
  • Ability to work flexible hours.

Benefit package available for staff who work an average of 30 hours or more per week. PTO accrual begins the day you start employment.
