Job description

Health and Safety Assistant:

Primary Duty:

The Health and Safety Assistant supports the safety, health, and overall stability of members within assigned communities. This role provides oversight of medication systems, health-related appointments, and documentation practices to support safe service delivery and program sustainability. The Health and Safety Assistant works in alignment with each member’s Person-Centered Service Plan (PCSP) and collaborates closely with supervisors, direct care staff, & leadership to identify and address health, safety, and documentation needs. This position maintains regular on-site presence across assigned programs, participates in on-call rotation, and communicates concerns, trends, and recommendations to program leadership to support continuity of care.

 

General Responsibilities:

  • Support health, safety, and medication systems across assigned programs in accordance with members’ Person-Centered Service Plans (PCSPs)assembly of seating system components
  • Ensure members meet annual appointments (physical, dental, optometry) expectations and documentation is uploaded into HER
  • Provide direct care support as needed to support program operations, including supporting medical and mental health appointments, transportation, and member needs.
  • Review and monitor medication systems, including Medication Administration Record (MAR) accuracy, medication changes, weekly eMAR reviews, monthly medication audits, controlled counts, storage, and disposal; collaborate with staff and supervisors to resolve concerns
  • Facilitate the ordering of medications and refills as prescribed and communicate with pharmacies regarding changes, discrepancies, or follow-up needs
  • Ensure allergies, warnings, and health alerts are accurately entered and updated in the electronic health record upon admission or change
  • Coordinate and monitor medical and mental health appointments, including scheduling, tracking, provider communication, documentation, transportation, and ensuring required routine and mandated appointments are completed
  • Participate and complete internal compliance reviews, as requested by CL Manager.  This may include, but not limited, to QA of service documentation, ensuring safety and medical items are stocked and maintained, safety protocols, and reviewing member’s financial documents

Requirements:

  • High school or GED/HSED;
    •  BA or BS degree in Human Services field and/or 2 years’ experience in Human Services are preferred.
  • Experience with medication administration, health coordination, or medical systems preferred; must successfully complete medication administration training 
  • Clearance from all applicable background checks 
  • Ability to pass a health assessment at time of hire and ability to maintain up to date health assessment as requested per regulations
  • Valid driver’s license
