FINGERPRINT REQUIREMENT - Remote process must allow 10 days, In Person can be done at Jackson or Grenada campus the week prior to start
Submission should include trauma level, bedsize and specific duties for each job in the past 3 years.

INTERVIEW PROCESS - leaders interview via webex (on screen interview) Please provide SPECIFIC times candidate is available to be on camera for a 20-30 min interview for the 72 business hours following submission.
THIS HIRING MANAGER IS LOOKING FOR HIGHLY EXPERIENCED CANDIDATES WITH ACUTE INPATIENT CASE MANAGEMENT AND DISCHARGE PLANNING EXPERIENCE
Training on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently.
**HARD REQUIREMENTS**
- Minimum 2 years of Acute Inpatient Medical Case Management experience that includes at least 1 year (in the last 3) in a LARGE BUSY Hospital ED (500+ beds, LEVEL 1/ACADEMIC Medical Center)
- Recent (within the last two years) Acute care discharge planning experience for patients with a wide range of medical diagnosis is REQUIRED,
- Heavy experience in telephonic, utilization management/review, insurance company experience, rehab, LTC, Hospice, is NOT the experience we are looking for.
- Previous travel in a level 1 facility (in the last 3 years)
- Strong computer skills, Must be proficient in EPIC charting
Shifts & Scheduling ? M-F 8-430p
Weekend Requirements None
Holiday Requirements Rotated as needed
Case load/# patients per day/consults per day 15-20
Will cover all Inpatient units throughout the hospital
RN's work with Social Work team which consists of 15 full-time and 17 part-time Social workers.
UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured.
UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.
Scrub Color/Dress Code Any color scrubs or business professional attire
RT for DC Planning? Will consult with RT as need
Pharmacy for DC Planning? Will consult with Pharmacy as needed
Department Specific Orientation 2 weeks (will include general facility orientation) Will work alongside CM team so will never be working alone.
