Job description

Kahak Health Care Services is a service provider for individuals with intellectual and developmental disabilities. Our mission is to foster independence through compassionate support, helping individuals to achieve their goals and enhance their quality of life. We provide in-home support services, community coaching, and respite care. As an employee of Kahak Health Care Services (KHCS) you will be a part of a team of skilled and dedicated professionals who value integrity, respect, and teamwork.


Duties of Diresct Support Professional (DSP):

The DSP provides direct care and support to individuals with intellectual and developmental disabilities (IDD) and/or behavioral health needs. The DSP assists individuals in achieving independence, improving daily living skills, participating in community activities, and maintaining health and safety in their home and community. This position requires compassion, professionalism, and a strong commitment to person-centered support. 


Duties of Sponsor / Sponsored Residential Provider:

This position takes place in your home. Sponsored Residential Providers serve as a supportive household member and mentor, offering structure, encouragement, and assistance while respecting the individual’s personal rights and independence. As a Sponsor you will:


  • Provide a safe, supportive, and welcoming home environment
  • Assist the individual with daily living activities, such as hygiene, meal preparation, and household routines
  • Support participation in community activities, social events, and recreational opportunities
  • Provide transportation to medical appointments, day programs, employment, and other community services
  • Promote skill-building and independence in areas such as:
    • personal care
    • budgeting and money management
    • cooking and cleaning
    • communication and social skills
  • Assist with medication support as trained and authorized
  • Monitor health, safety, and overall well-being
  • Maintain accurate documentation, logs, and reports as required
  • Follow the individual’s Individualized Service Plan (ISP)and agency guidelines
  • Communicate regularly with the care team, family/guardian, and agency staff
  • Support the individual during emergencies and ensure safety protocols are followed


Requirements of Both Positions: A minimum of one year experience in the medical or a related field (social work, psychology, nursing assistance, etc.) and a high school diploma or equivalency. Must be 18 years of age. Additionally, the following are required:

  • A reliable car
  • A current driver's license
  • A current work authorization (US Passport, US Work Authorization card, Permanent Resident Card) or a Birth Certificate
  • Social Security Card
  • Current auto insurance
  • Current CPR & First Aid
  • Current physical exam
  • Current tuberculosis test with negative result
  • Resume

If hired, background checks (FBI) and a Central Registry (CRS) check will be required.
