Job Title: Assistant Case Manager

Job Summary

This is a paraprofessional position that performs a variety of case management duties for Medicare/Commercial Advantage Plans. Job functions include coordinating medical appointments and providing managed care agencies with clinical updates as requested.

Essential Job Functions

Review all Medicare Advantage (Managed Care/HMO) admissions and discharges daily.

Assist case manager in coordinating follow up medical appointments

Review all pending medical appointments and follow up with nursing units and transportation department.

Provide clinical updates and other documentation requested by Managed Care agencies.

Coordinate equipment and services for pending discharges.

Provide bed management oversight, and other duties as needed

Job Requirements

Education:

High School Diploma or equivalent. Associate degree preferred.

Experience:

One-year experience working in a community service agency that provides service to elderly clients preferred

Licenses/Certifications:

N/A

Abilities Required

Sensitivity and skill required in working with impaired elders and families by phone

Must be organized, able to work independently and understand confidentiality which is necessary for this program.

Must be bi-lingual (English/Spanish)
