We are launching an innovative pilot program aimed at managing post-hospital discharge patients through a hybrid model that integrates in-person consultations, virtual visits, and telehealth services. The client is in search of a Physician Assistant (PA) who is enthusiastic about contributing to a dynamic program with significant long-term prospects.

Key highlights of this opportunity include:

- A newly established program designed for expansion and development

- Outpatient care model with no on-call responsibilities
