Full-Time Administrative LPN

Monday - Friday 8 am to 4 pm

DUTIES:

  • Assist in creation of plan of care for all new individuals served
  • Conduct supervisory visits and document results
  • Complete and update health, risk and safety assessments using the Health Risk Screening Tool (HRST)
  • Review of data tracking and documentation
  • Conduct proxy care training
  • Provide staff training and oversight
  • Supervision of LPN Staff
  • Coordinate medical visits for clients
  • Fill in on medication passes as necessary
  • Other duties as assigned
