Description
Coverage Territory: Champaign and surrounding areas
Schedule: 10 hour shifts Rotating weeks: Mon, Tues, Fri, Sat & Sun, Wed, Thurs, Fri
Be There When It Matters Most
At Transitions Hospice, we don't just manage care - we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means someone is always present - through the quiet hours of the night - so patients feel safe, families can rest, and no one faces the darkness alone.
If you're a CNA who understands that nighttime presence is its own kind of care, this is your role.
What You'll Do
As a Companion Aide CNA on the night shift, you'll be a steady, compassionate presence at the bedside - monitoring patient safety, providing personal care, and giving families the peace of mind that someone is watching over their loved one through the night. Your shift includes:
Overnight Patient Care & Safety
  • Providing continuous bedside presence and patient supervision throughout the overnight shift to ensure safety and comfort
  • Monitoring patient condition, behavior, and environment - and promptly reporting any significant changes to the on-call nurse
  • Preventing falls, elopement, or self-harm through attentive observation and appropriate intervention within CNA scope of practice
  • Responding promptly to patient requests and ensuring a calm, restful overnight environment
  • Assisting with personal care including repositioning, toileting, bathing, hygiene, and dressing as assigned
  • Providing proper skin care and observation to prevent tissue breakdown during prolonged periods of rest
  • Supporting normal range of motion and repositioning to maintain patient comfort and skin integrity

Observation & Documentation
  • Observing and documenting patient condition, sleep patterns, behaviors, and notable changes throughout the shift
  • Taking and recording vital signs - temperature, pulse, respirations, blood pressure, and weights - when ordered and within demonstrated competency
  • Reporting significant changes in patient condition to the RN or on-call supervisor promptly and without delay
  • Adhering to the organization's documentation standards and professional conduct expectations

Family Support & Communication
  • Providing families and caregivers with reassurance and overnight respite support
  • Maintaining respectful, ethical communication with patients and families, including strict confidentiality
  • Demonstrating comfort and compassion in providing specialized care to terminally ill patients and their families

Safety & Equipment
  • Using patient equipment safely and appropriately - including foot stools, side rails, and oxygen
  • Assisting with ambulation, exercise, and range of motion as directed by the hospice nurse or therapist
  • Participating in quality assessment and performance improvement activities

Benefits We Offer
  • Competitive pay
  • Medical / Dental / Vision Insurance plans
  • Long & Short-Term Disability
  • 401(k) with company match
  • Auto Allowance & Gas Card
  • Company provided equipment
  • Paid Time Off

Why Choose Transitions Hospice?
CNAs at Transitions aren't just sitters - they're a vital part of the care team. The overnight hours can be the most vulnerable for patients and families, and our Companion Aides show up with skill, calm, and compassion when it matters most. We give you the support, training, and team to do your best work - even in the quiet of the night.

What You Bring
  • Current CNA certification meeting state training requirements
  • Current CPR certification
  • 1+ year of hospice, home health, or related healthcare experience preferred
  • Prior experience in overnight or night shift patient care preferred
  • Ability to work independently and remain alert and attentive throughout overnight hours
  • Strong observational skills with the ability to recognize and report changes in patient condition
  • Genuine compassion for patients and families navigating end-of-life care

Salary Description
$19.00 - $23.00 an hour
