Job description

Registered Nurse Home Health PRN Tacoma

This position is paid on a per-point basis.

Schedule: Monday - Friday 8:00 a.m.- 5:00 p.m./PRN as needed

Responsibilities:
  • Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.
  • Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).
  • Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.
  • Offer education and emotional support to patients and their families/caregivers based on individual needs.
  • Supervise LPNs/LVNs, home health aides, and other support staff.
  • Participate in on-call rotations, admissions, or urgent response as needed (common in hospice roles).
  • Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations.
  • Monitor patient progress, respond to changes in condition, and adjust care plans accordingly.
  • Maintain accurate clinical records and ensure care aligns with agency policies, professional standards, and regulatory requirements.
Qualification:
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
Required Certifications and Licensures:
  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
