Role Overview

Nanoom Med is seeking a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing outpatient practice. Candidates with fluency in Korean, Mandarin, Cantonese, or Japanese - or those with strong cultural familiarity and experience serving East Asian communities - are highly encouraged to apply.

As part of a close-knit clinical team, you will provide comprehensive primary care services, manage chronic conditions, and support preventive health initiatives. This role offers the opportunity to build meaningful relationships with patients from East Asian backgrounds, many of whom are seniors, while practicing in a supportive and collaborative environment.

Providers in our practice work both independently and alongside physicians to ensure continuity of care and excellent patient outcomes.

Responsibilities:

  • Provide comprehensive primary care services in an outpatient setting.
  • Conduct routine office visits and address acute medical concerns.
  • Perform annual wellness visits and health risk assessments.
  • Educate patients on their conditions, treatment plans, and preventive care strategies.
  • Coordinate care with specialists, diagnostic facilities, and other healthcare providers.
  • Utilize telemedicine as appropriate for patient follow-up and consultation.
  • Participate in care team discussions, ensuring best practices in patient management.
  • Maintain accurate, thorough documentation of patient encounters.
  • Engage in population health initiatives, including routine screenings and preventive care measures.
  • Other duties as assigned.

Qualifications:

  • Certified Nurse Practitioner (NP) or Physician Assistant (PA) with experience in primary care (internal medicine, family medicine, or geriatrics preferred).
  • Fluency in Korean, Mandarin, Cantonese, or Japanese is strongly preferred.
  • Candidates with significant experience serving East Asian patient populations are highly encouraged to apply.
  • Experience with Medicare patients and senior-focused care is a plus.
  • Strong understanding of medical documentation, coding (ICD-10, HCC), and risk adjustment.
  • Active, unrestricted state license and DEA registration.
  • Medicare enrollment in good standing or eligibility to enroll.
  • Passion for primary care and patient-centered medicine.
  • Strong teamwork and collaboration skills.
  • Ability to adapt to a dynamic clinical environment and embrace technology in patient care.
