Physical Therapist

Job Type: Per Diem (PRN)

Setting: Home Health

Arbor Home Health Therapy provides home health agencies with staffing for Physical Therapy, Occupational Therapy and Speech Therapy. Our partnerships with each home health agency will determine the documentation system in which you will document your visits (Therapy Boss, Kinnser or Home Care Home Base).

We are currently seeking a Physical Therapist to provide physical therapy services in the community and AL settings.

The Home Health Therapist must be:

  • Self-motivated, organized, and responsible for patient care including scheduling of appointments.
  • Timely with documentation of visits and communication with the Clinical Program Manager and therapy team.
  • Comfortable entering patient homes in the community, as well as Independent & Assisted Living facilities.
  • Willing to commit to following a patient throughout the POC for initial, interim and discharge visits.
  • Routine follow up visits will typically be assigned to therapist assistants (PT/OT), and the evaluating therapist is responsible for cosigning treatment visit notes on a weekly basis.
  • Rate of pay is based on the type of visit assigned and paid at a flat, per visit rate (Range from $55-$90/visit).
  • Reliable transportation and willingness to travel within a 45-mile radius from home is expected.

Workplace Behaviors:

  • Demonstrates a helpful, positive attitude with all patients and caregivers.
  • Demonstrates ability to organize and manage patient schedules, provide functional interventions based on the POC, manage time for each visit and complete documentation within 48 hours of visit.
  • Dresses in scrubs or business casual attire to represent Arbor Home Health in a professional manner.
  • Reports directly to Home Health Clinical Program Manager all missed visits, change in patient condition warranting agency involvement (i.e., hospitalization, falls, etc.).
  • Reviews respective documentation systems daily to correct/resubmit outstanding documentation.
  • Maintains current licensure in good standing.
  • Therapist must have personal Home Health Kit including thermometer, blood pressure cuff & stethoscope (automated device acceptable), and pulse oximeter.
  • PPE can be provided upon initial hire; however, it is expected to be replenished by therapist thereafter.

Requirements:

  • Valid Physical Therapy License
  • CPR Certification
  • Excellent observation, verbal and written communication skills, critical thinking skills, basic math skills.
  • Compassion, patience, and a positive attitude.
