APS Investigators often encounter emotionally challenging situations, such as working with vulnerable adults who may suffer from neglect, abuse, or exploitation. Navigating complex family dynamics and ensuring the safety of clients can be stressful, especially when facing resistance or lack of cooperation. Additionally, heavy caseloads and strict documentation requirements can contribute to a demanding workload. However, working within a supportive team environment and accessing ongoing training can help address these challenges while making a meaningful difference in the lives of at-risk adults.
To excel as an APS (Adult Protective Services) Investigator, you need a background in social work, criminal justice, or a related field, with knowledge of investigative procedures and laws regarding adult abuse and neglect. Familiarity with case management software, government databases, and sometimes state-mandated certifications or licensure are important. Excellent communication, critical thinking, empathy, and conflict resolution skills distinguish outstanding investigators in this field. These competencies are vital to effectively assess complex situations, protect vulnerable adults, and collaborate with other agencies to ensure client safety.
An APS (Adult Protective Services) Investigator is responsible for investigating reports of abuse, neglect, or exploitation of vulnerable adults, such as the elderly or individuals with disabilities. They assess the safety of clients, interview involved parties, collect evidence, and collaborate with law enforcement or social services to ensure appropriate interventions. Their goal is to protect at-risk adults and connect them with necessary resources to improve their well-being.

The QA Coordinator plays a critical role in supporting accurate data collection, incident reporting, and continuous quality and performance improvement as an integral member of the Hospital's Quality Assurance and Performance Improvement (QAPI) team. This position is the primary individual responsible for processing and submitting Public Records Requests, ensuring all redactions comply with HIPAA, regulatory guidelines, and State requirements. The QA Coordinator supports risk management functions through incident report coding, video reviews, and the preparation of Hospital committee agendas and minutes. The role also assists in developing required documentation for the Hospital's Independent Oversight Committee (IOC) and collaborates closely with the Patient Rights Advocate (PRA) to address patient concerns and support Adult Protective Services (APS) investigations when necessary. Additionally, this position supports regulatory readiness by compiling and preparing incident-related documentation for surveys conducted by the Arizona Department of Health Services (ADHS) and The Joint Commission (TJC).
Review, code, and classify incident reports based on type, severity, and relevant Quality of Care (QOC) indicators to support accurate data tracking, trend analysis, and regulatory compliance. Ensure consistent application of standardized coding guidelines. Collaborate closely with the QA Specialist on incident report coding and video review processes, and document all incidents requiring video review in the Video Tracker Log daily. Follow-up on pending incident report reviews.
Collect and process patient-specific Public Records Requests, including Incident Reports, Complaints, Grievances, Appeals (CGA), and Hospital policies. Track all requests using the Hospital's tracking system and submit them to the Office of Administrative Counsel & Rules (OACR) with appropriate HIPAA-compliant redactions in accordance with Hospital policy and regulatory requirements. Conduct follow-up with OACR every 30 days until requests are fulfilled and keep patient's apprised.
Assist the Hospital's Patient Rights Advocate (PRA) in addressing patient concerns and, when appropriate, submitting reports to Adult Protective Services (APS) and the Arizona Department of Health Services (ADHS) related to allegations of abuse, neglect, or exploitation. Support and coordinate on-site APS investigations as needed.
Prepare and submit daily Executive Risk Management Team (ERMT) meeting minutes, summarizing incidents and providing them to the Chief Quality Officer (CQO) each morning. Additionally, record and submit monthly minutes for Hospital QAPI committee meetings including Zero Harm, Utilization Management, and Quality Council.
Assist the Hospital's Performance Improvement (PI) Specialist with data collection, tracking, and trending, and support performance improvement projects and initiatives.
Collect and review Seclusion & Restraint (S&R) packets, ensuring S&R packet supports the submitted incident report. Complete S&R packet audit, in collaboration with medical records, to report compliance during monthly Hospital committees.
