Registered Nurse – Case Manager

Position Overview

We are seeking a compassionate and detail-oriented Registered Nurse – Case Manager to join our acute hospital team. The RN Case Manager will play a critical role in care coordination, discharge planning, and utilization review, ensuring patients receive safe, efficient, and high-quality care across multiple hospital campuses.

  • Conduct comprehensive needs assessments and coordinate durable medical equipment (DME) orders.
  • Develop, implement, and monitor individualized plans of care.
  • Facilitate discharge planning and transitions of care.
  • Collaborate with interdisciplinary teams to ensure effective care coordination.
  • Perform utilization review in compliance with CMS (Centers for Medicare & Medicaid Services) regulations.
  • Ensure adherence to hospital policies, procedures, and regulatory standards.
  • Float between units as needed, including Colby and Pacific campuses (within 2 miles).

Required Qualifications

  • Active RN Licensure (Washington State).
  • BLS Certification (Basic Life Support).
  • Minimum 2 year of acute care experience.
  • Experience in discharge planning and care coordination.
  • Strong preference for candidates with diverse case management experience across:
    • Medical-Surgical
    • Long Length of Stay (LLOS)
    • Emergency Department (ED)
