We are searching for a REACH Care Coordinator!
Requirements
Applicants must have a vehicle and auto insurance, a bachelor's degree in a relevant area of education or human services, at least one year of experience in the mental health field, basic computer knowledge, excellent writing skills and interpersonal communication skills, strong organizational and time management skills, and a welcoming demeanor and positive attitude. Care Coordination is a robust position that involves helping children & youth with mental health needs, and their families, so that children & youth can remain safely in the community. A Care Coordinator must have outstanding organizational and time management skills along with very strong written and oral communication skills.
Job duties include:
  • Conducts weekly face-to-face visits with each youth and family in the community or at clients' homes. *
  • Facilitates monthly team meetings with the Child and Family Team for each youth on caseload.*
  • Utilizes Synthesis online electronic medical records software to complete necessary paperwork including but not limited to Plans of Care documents, court letters, and daily provider notes.*
  • Completes referrals for services that the Child and Family Team has deemed appropriate.*
  • Attends court hearings for all youth on caseload.*
  • Engages with youths' schools to ensure academic needs are met.*
  • Participates in weekly supervision with supervisor and monthly consultation with agency psychologist.*
  • Monitors services that are in place to ensure quality care is being given to each youth and family.*
  • Keeps and monitors personal crisis phone throughout the work week as well as keeps and monitors APC agency crisis phone two to three weekends per year and responds as needed to crisis situations with youth and families.*
  • Participates in monthly program in-services and trainings.*
  • Participates in weekly APC Care Coordination meetings.*
  • Assists with APC agency events and functions. *
  • Other duties as assigned. *
