Job description

Join Our Growing Cardiology Team in Beautiful Space Coast, Florida!

Health First is seeking an experienced Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) specializing in Heart Failure to support our expanding cardiology services in Brevard County, FL.

This dedicated clinic role is ideal for a seasoned clinician looking for balanced patient volumes, collaborative physician support, and a structured, high-impact clinical focus.

Position Highlights

  • Manageable Workload: 1215 patients per day allowing for high-quality, comprehensive patient interactions.
  • 100% Outpatient / Strictly Clinic: Dedicated clinic setting focused on long-term disease management and post-hospital follow-up.
  • Strong Collaborative Support: Work closely alongside our dedicated Cardiologists.
  • Work-Life Balance: Answering service in place for after-hours calls from established patients only.

Key Responsibilities

  • Provide specialized clinical care for heart failure patients in an outpatient setting.
  • Conduct targeted post-hospital discharge follow-ups to optimize clinic transitions and reduce readmissions.
  • Manage complex chronic heart failure medical therapies and patient education.
  • Collaborate directly with supervising/supporting cardiologists on care plans and escalations.

Qualifications

  • Active (or eligible) Florida APRN or PA license.
  • Minimum 12+ years of clinical experience in Cardiology or specialized Heart Failure management required.
  • Strong diagnostic and clinical decision-making skills in outpatient chronic disease management.
