As part of our hospice center, you will engage with a variety of healthcare professionals, including physicians and nurses, to develop and implement holistic treatment plans for patients.

Key Responsibilities

As a Licensed Clinical Social Worker, you will have a multifaceted role that includes the following key responsibilities:

  • Offer individual therapy sessions to adult clients experiencing chronic pain, anxiety, depression, and trauma related to workplace injuries.
  • Facilitate weekly psychoeducational class sessions, providing valuable information and coping strategies to support clients' recovery.
  • Conduct brief therapeutic sessions for individuals enrolled in the Functional Rehabilitation Program (FRP), monitoring their clinical progress.
  • Perform detailed assessments and develop comprehensive treatment plans for an active caseload, documenting progress in our Electronic Medical Records (EMR) system.
  • Deliver clinical care that is informed by evidence-based practices in pain psychology and adheres to legal and ethical guidelines.
  • Collaborate with other healthcare professionals and refer clients to additional services as necessary to enhance treatment outcomes.
Patient Population / Client Focus

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited university.
  • An active California license as a Clinical Social Worker (LCSW) is required.
  • A minimum of 2 years of clinical experience in settings involving Cognitive-Behavioral Therapy, Acceptance and Commitment Therapy, and mindfulness-oriented interventions.
  • Proficient in assessing and treating patients with chronic pain conditions.
  • Demonstrated ability to work effectively with a trauma-informed, integrated therapeutic approach.
  • Excellent verbal and written communication skills, with a focus on establishing rapport and connecting with clients and their families.
  • Strong judgment and decision-making skills to support patient care.
