The Case Manager provides quality case management, psychotherapeutic services, and discharge planning to all patient and their families, serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and their families.

Requirements
Education & Experience:
โ€ข Bachelors Degree in Social Work, Psychology or Social Services
โ€ข Master's Degree preferred
โ€ข 3 years behavioral health experience preferred
โ€ข Associate license such as ASW (Associate Clinical Social Worker), AMFT (Associate Marriage & Family Therapist), APCC (Associate Professional Clinical Counselor) preferred
โ€ข Current BLS healthcare provider card required upon hire
โ€ข Therapeutic Optionsโ„ข training to be obtained prior to providing direct patient care without a preceptor
