We are currently seeking a Licensed Independent Clinical Social Worker (LICSW) to serve as a Medical Social Worker (Special Needs Coordinator) at McChord Airmen Clinic on Joint Base Lewis-McChord.
Position Summary
As the Special Needs Coordinator, you will provide care coordination and case management services for military families by identifying patients with special medical or educational needs, coordinating care plans, and supporting enrollment in military healthcare programs.
Key Responsibilities
- Identify patients and family members requiring special medical or educational support
- Conduct Exceptional Family Member Program (EFMP) assessments for eligible military families
- Perform comprehensive patient assessments and ongoing care coordination
- Develop and monitor individualized care plans in collaboration with providers, patients, and caregivers
- Coordinate the Family Member Relocation Clearance process
- Provide clinical social work services in accordance with professional standards and ethical guidelines
- Assist families with enrollment in TRICARE and Extended Health Care Options (ECHO)
- Maintain accurate documentation and communicate effectively with multidisciplinary healthcare teams
Minimum Qualifications
- Master's Degree in Social Work (MSW) from a CSWE-accredited program
- Current, unrestricted Licensed Independent Clinical Social Worker (LICSW) license
- Minimum of 2 years of full-time experience in care coordination, discharge planning, or case management in a medical setting within the past four years
- Current American Heart Association or Red Cross Basic Life Support (BLS) certification or willing to get one.
- U.S. Citizen
- Ability to successfully complete a National Background Investigation
- Reliable transportation to and from the work site

- Strong case management and care coordination skills
- Excellent communication and documentation abilities
- Ability to work collaboratively within an interdisciplinary healthcare team

- 7:30 AM to 4:30 PM
