The Opportunity:
One Medical is looking for nurses with care management or population health backgrounds to join our Senior Health Care Team. In this role, you will support One Medical senior patients across the care continuum through scheduled visits and asynchronous proactive outreach and care coordination. You will be based in an office where seniors are seen and support patients across multiple offices.
Employment type:
  • Full time 40 hours, Monday-Friday in office

What you'll be working on:
  • Conduct independent nursing visits (in-person, video, remote) with patients focusing on prevention (including Annual Wellness Visits), medication reconciliation, geriatric assessment, care plan reinforcement, chronic disease follow-up, and advance care planning
  • Support patients to be more effective at managing their own care, navigating the healthcare system, and understanding their medical conditions and medications
  • Coordinate with primary care providers and health navigators on follow-up care before and after clinician visits to deliver high quality, whole-person care, with a particular focus on high risk patients)
  • Attend case conferences and team huddles to support collaborative patient care
  • Coordinate with community resources including home health, specialists, durable medical equipment vendors, and others to deliver high quality care and support patients
  • In partnership with other team members, identify and act on open care gaps including cancer screenings, hearing/vision assessments, metabolic health, vaccines, functional status evaluations, fall risk, and cognition/mental health screenings
  • Collaborate with and provide clinical support for Health Navigators

Education, licenses, and experiences required for this role:
  • Licensed RN in CA required, willingness to obtain licenses in other states
  • Bachelor of Science in Nursing or MSN preferred
  • 2+ years of nursing experience in hospital, post-acute facility, or ambulatory setting
  • Working in office with primary care providers and health navigators
  • Experience preferred in one or more the following: care management, value-based care, population health
  • Working 8am-5pm Pacific Time, knowledge of the greater Bay Area healthcare landscape and geography or willingness to rapidly learn

  • Must demonstrate outstanding clinical aptitude, using sound judgment in caring for patients
  • Must demonstrate ability to use core coaching and educational techniques, including motivational interviewing and patient-centered communication to activate and empower patients and families
  • Must demonstrate excellent interpersonal communication skills with a variety of audiences via in-person, telephone, video, and electronic means including exceptional listening skills, ability to use appropriate language and demonstrated writing skills. Comfortable engaging speciality clinicians and other members of the healthcare team
  • Must demonstrate ability to think outside of standard operational structure and work autonomously
  • Must be creative, flexible, and organized to focus on continuous-improvement and be comfortable with ambiguity and change

This is a full-time in office role. Location is Strawberry Village with being able to commute to San Rafael location.

The base salary range for this role is $113,000 to $120,000 per year based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience, physical location (state you live in) and patient care schedule.
