COPC is the largest independent physician-owned primary care group in the nation. They are seeking an experienced full-time licensed board-certified NP/PA-C to join their team at Canyon Medical Center, 8050 E. Main, Suite 2200, Reynoldsburg, OH 43068.

About the area:

This practice is about twelve miles from downtown Columbus and about a 20-minute drive from neighboring towns.

Columbus can be compared to a small yet big city. Higher education is strong in Columbus, with our university ranked in the top 25. The arts community offers numerous festivals and events, galleries, and cool exhibits. There are over 500 parks in Columbus. Ongoing entertainment venues and many popular sports teams, including well-known football, hockey, and soccer teams, are here. Golf is a favorite pastime in Columbus with several golf courses in the town and dozens in neighboring communities. If you like to shop, Columbus offers endless retail, and there is a great variety of restaurants. It's a foodie town. About the practice:

This well-established practice has 10 PCPs, an Office Manager, and several medical assistants and service reps to support the providers and patients.

This Primary Care Group is an outpatient practice with a great reputation for patient care in the community. Its focus is on health education, preventive medicine, and disease management to deliver high-quality patient care that is the Best for Primary Care.

COPC uses state-of-the-art EHR - EPIC. The company offers training during new hire orientation.

Office Hours: 8:00 - 4:30 pm Mon-Friday.

Key Responsibilities:

Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.

Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.

Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies.

Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.

Collaborate with physicians, advanced practice providers, nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.

Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.

Qualifications:

Master’s degree in nursing or physician studies from an accredited program.

Current licensure as a Nurse Practitioner in Ohio.

National certification as a Family Nurse Practitioner (FNP).

Preferred: A minimum of 1 year of practical working experience as an NP post-graduation in an outpatient practice setting.

Strong clinical assessment, diagnostic, and treatment skills.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a multidisciplinary team.

EPIC EHR experience is helpful. Training is available at onboarding.
