As a Physical Therapist in our home care facility, you will work onsite in a supportive and collaborative environment designed for optimal patient engagement. Our facility prioritizes patient-centered care, encouraging communication and teamwork among staff. The standard work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, allowing for a balanced professional and personal life.

• Document clinical notes consistently and accurately in accordance with the organization's policies, ensuring compliance and facilitating effective communication within the healthcare team.
• Provide high-quality, individualized patient care during therapy sessions, integrating evidence-based practices tailored to each patient’s treatment plan.
• Encourage and motivate patients through clear communication, assisting them in understanding and committing to their therapy goals.
• Utilize time management skills to balance caseloads efficiently, ensuring that each patient receives the attention necessary for effective rehabilitation.
• Engage in regular collaboration with physicians and other healthcare professionals to optimize patient outcomes and create a holistic treatment approach.

Your primary focus will be on orthopedic outpatient clients, ranging from post-surgical recovery to injury rehabilitation. You'll work closely with patients of various demographics, tailoring treatment ideas and exercises based on their unique physical abilities and recovery goals. • Possession of a Bachelor’s Degree in Physical Therapy or a higher degree from a recognized and accredited institution.
• A valid and active California Physical Therapist license, in good standing, is essential.
• A minimum of 2 years of previous clinical experience is required, showcasing a strong passion for patient-centered care and rehabilitation.
• Proficiency in manual therapy techniques and familiarity with electronic medical records (EMR) systems, particularly WebPT, is preferred.

While a minimum of 2 years in practice is required, candidates with advanced credentials such as certifications in specialized physical therapy methodologies or experience with specific patient populations may be given preference. Additionally, familiarity with community outreach programs will be considered a valuable asset.
