About the Facility
Ahava Therapy Services is seeking a Full-Time Physical Therapist to join our in-house rehab team at Ahava Healthcare of Clarksville, a 122-bed skilled nursing facility offering short-term, long-term, and outpatient services.

Responsibilities
  • Review resident information and evaluate its impact on assessment and treatment.
  • Conduct and document thorough functional assessments using standardized procedures.
  • Develop and implement individualized treatment plans with achievable goals.
  • Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
  • Instruct and supervise therapy assistants as needed.
  • Enter treatment and progress notes for each session.
  • Revise treatment plans as needed throughout the course of care.
  • Document treatment in clear, concise discharge summaries.
  • Orient residents to services and treatment procedures appropriate to their needs.
  • Collaborate with nursing staff, residents, and families to maximize functional abilities.
  • Educate residents and caregivers on home exercise and therapy continuation.
  • Assist with referrals for additional services as needed and maintain knowledge of community resources. Participate in rehabilitation team conferences and other
  • facility meetings.
  • Access continuing education opportunities to maintain clinical competency.
  • Participate in quality improvement initiatives.
  • Follow regulatory, professional, and facility standards.
  • Perform other duties as assigned to provide quality care.
  • Understand payer reimbursement and allocate therapy resources efficiently.
  • Support Ahava's mission and project a professional image.
  • Maintain expected productivity levels.
Qualifications
  • Bachelor's or Master's degree in Physical Therapy
  • Current PT license in the state of practice
  • BLS/CPR certification (current or obtained during orientation)
  • Strong communication, teamwork, and organizational skills
