Clinical Intake Liaison (LPN) Position Summary

As a Clinical Intake Liaison (LPN) at Ahava Healthcare, you'll be an essential part of our referral process. Responsibilities include:

  • Review all referrals in a timely manner. Maintain a working knowledge of community’s practices related to payor and hospital contracts.

  • Work alongside the insurance liaison for review.

  • Communicate with referral sources regarding referral status and communicate with the facility to coordinate admissions.

  • Maintain confidentiality of protected health information, including verbal, written and electronic communications.


Clinical Intake Liaison (LPN) Requirements

  • Current unrestricted LPN license for the state of Tennessee

  • Experience in a nursing or Admissions position in a health care facility

  • Experience in SNF preferred
