Job description

Under the supervision of the Multipurpose Senior Service Program (MSSP) Supervising Care Manager (SCM), the Care Manager Aide (CMA) I provides assistance to the Social Work Care Managers (SWCM). MSSP serves frail low-income elders by planning and coordinating a wide range of services to assist clients in avoiding nursing home placement and to remain living independently in a safe manner. All work will be performed within established procedures under the direction of the SWCM, SCM or Nurse Care Manager (NCM). The MSSP Care Manager Aide I works as a member of the Choice in Aging (CiA) team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity in community.
Essential Job Functions:
Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.

Job functions include but are not limited to the following:
  • Assist in coordinating services for socially and medically complex clients with guidance of SWCMs.
  • Monitor health and/or psychosocial changes in clients through monthly contacts with client, family, and professionals as needed.
  • Participate in case conferences.
  • Run Medi-Cal eligibility for new referrals to determine whether Medi-Cal code is appropriate.
  • Enter new referral information into MSSP Care/database if they have an eligible MSSP code.
  • Open client cases in MSSP Care/database for nurse care managers.
  • Maintain LOC/ ADV monthly reports for nurse care managers.
  • Plan termination of clients using MSSP protocol for whom services are no longer necessary or appropriate.

Minimum Qualifications:
  • Two years of experience working with the elderly.
  • The ability to communicate in English in both written and verbal forms.
  • Must have valid California Driver’s license, clean DMV record, and automobile insurance.
  • Skills to manage care for vulnerable crisis-prone clients with diverse medical and social problems.
  • Computer literacy to input data on electronic data management system.
  • Current first aid/CPR certification required.
  • Must pass Department of Justice fingerprint clearance.
