Responsibilities
- Provide personal care and assistance with activities of daily living (bathing, dressing, feeding, mobility)
- Companion and supervision support in the clientโs home or facility
- Monitor client health status and report any changes immediately to the coordinator
- Document activities and observations in WellSky
- Maintain active and timely communication with the scheduling team
Mandatory Requirements โ

Requirement Detail
Valid driverโs license Required โ no license, no further process
Reliable personal transportation Must be able to travel between clients
Eligibility to work in the U.S. Documentation required to complete the I-9
Schedule availability Morning, afternoon, night shifts, and weekends
Social Security Number (SSN) Required for background check and payroll
Active cell phone For communication with the team and clients
Required Documents for the Hiring Process ????
All candidates must submit the following documents through the application portal:
Identity & Work Eligibility (I-9)
- Valid government-issued photo ID (e.g., driverโs license, passport, state ID)
- Document proving U.S. work eligibility (e.g., Social Security card, U.S. passport, EAD)
Tax Documents
- W-4 Form (federal tax withholding) โ completed during onboarding
Background Check
- Signed background check authorization โ mandatory before starting any shift
Onboarding Documents
- Signed company policies and employee handbook
- Direct deposit banking information (optional but recommended)
Certifications
- CNA, HHA, or CPR/First Aid certification (if applicable)
- At least 1 professional reference from a previous caregiving role
