• Conduct comprehensive psychosocial assessments utilizing interviews, observations, and medical record reviews to evaluate patients' emotional and social factors that may affect their recovery.
• Provide individual and group counseling to patients and their families, helping them navigate adjustment issues stemming from illness and disability, while fostering resilience and coping strategies.
• Collaborate with an interdisciplinary rehabilitation team to create tailored treatment plans that reflect the unique psychosocial needs of each patient.
• Assist patients in accessing valuable community resources, such as vocational rehabilitation services, support groups, and housing assistance.
• Educate patients and their families regarding post-rehabilitation services, ensuring they are equipped with the knowledge to thrive post-discharge.
• Participate actively in discharge planning to ensure smooth transitions for patients returning home or moving to other care settings.

The patients you will serve come from diverse backgrounds and experiences, often facing challenges related to rehabilitation following trauma, chronic illness, or mental health issues. Your empathetic support will be pivotal in guiding them through their recovery process, with a focus on emotional well-being and practical assistance. Our clinic caters to individuals requiring rehabilitation due to a variety of conditions, including neurological disorders, orthopedic surgeries, and the aftermath of strokes.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Exceptional communication skills, with the ability to build rapport and trust with patients and their families.
• Strong analytical skills to assess the psychosocial determinants affecting health and recovery.

• Previous experience in a rehabilitation or healthcare setting with demonstrated knowledge of adjustment issues, grief, and coping mechanisms.
• Familiarity with community resource networks and how to effectively assist patients in accessing those resources.
• A background in working within multidisciplinary teams, contributing to a collaborative and supportive environment.
