Founders RX is a closed-door pharmacy servicing the needs of Skilled Nursing Facilities around the state of Colorado. We are seeking an individual that is passionate about healthcare and patient well-being, with a keen understanding of medications and their proper use.

Founders RX is seeking a reliable and detail-oriented Pharmacist to join our team and play a crucial role in delivering quality pharmaceutical services to our valued customers.

We are seeking an After Hours/On-Call Long-Term Care Pharmacist to support our LTC pharmacy operations. This role is ideal for a detail-oriented pharmacist who is passionate about patient safety, regulatory compliance, and collaborative care within long-term care facilities. The position offers part-time hours with a long-term opportunity.

Schedule: On-Call/After-Hours

Member will be on call for a minimum of 4 hours/night at pharmacy close (8pm) for any after hour needs.

Essential Functions and Potential Responsibilities:

  1. Review, verify, and dispense medications for long-term care residents
  2. Conduct medication regimen reviews (MRRs) and ensure clinical appropriateness
  3. Ensure compliance with federal and state pharmacy regulations, including LTC-specific requirements
  4. Collaborate with physicians, nurses, and facility staff to resolve medication-related issues
  5. Oversee accurate documentation and proper medication labeling and packaging
  6. Provide clinical support and consultation as needed
  7. Support quality assurance initiatives and continuous improvement efforts

Qualifications

  1. Active Pharmacist license in Colorado.
  2. Previous experience in Long-Term Care pharmacy strongly preferred but not necessary.
  3. Strong knowledge of LTC regulations, medication management, and clinical best practices.
  4. Excellent attention to detail and organizational skills.
  5. Ability to work independently.
  6. Proficiency with pharmacy software systems (LTC systems are a plus).
